It means, if a blogger got 40,000 pageviews per month, they he is making roughly $400 a month. If he got 400,000 pageviews per month, the income would be about $4000 a month. Definitely it could be visitors from USA. Also it is written that it was through display ads which firms used to give. But this changed now also.
